Managed
care has made health insurance considerably more complicated
than it used to be. Our office staff will do everything possible
to try to make this process go smoothly for you and to assist
you in getting your claim paid.
There are
several specific actions you can take that will help us do this
better:
1) Complete
your preregistration online Click Here!
2) Verify
that the doctor you are scheduled to see in The Hand Center
is on your plan.
3) If a
referral is required from your PCP (Primary Care Physician),
please be sure that this has been done by letter, phone, or
fax.
4) Bring
appropriate documentation with you to The Hand Center
a) Current
Insurance Card
b) Valid Driver's license or Picture ID
5) All
patients under the age of 18 must be accompanied by a parent
or legal guardian.
a) If
someone other than a parent or legal guardian is to bring
a minor to the office, we will need a signed Medical
Consent form. You may download this form from our website.
6) If the
patient is hearing impaired or needs translation in some language
other than Spanish, and if the patient is not bringing an interpreter
with him or her, please notify us in advance so that we may
arrange for an interpreter.
Other Hand
Center Policies regarding insurance:
1) We
do not bill 3rd party insurance. This specifically refers to
motor vehicle accidents in which another person's insurance
company is paying. You will be responsible for this bill.
2) Patients who are seen here at the referral of an attorney for
cases involving 3rd party liability, payment in full is due at the time of service.
3) Worker's
Comp. injuries more than 6 months old require that the previous
medical records be available for review by the Hand Center doctors.
(This will often save having to repeat certain tests).
4) At the
time of your visit, you will be asked to sign a Disclosure of
Information form. This will allow us to obtain any previous
medical records not available at the time of your visit.
